Management of Acute Asthma Attack

O SHIT ME!
 

Oxygen 

Salbutamol (2.5 - 5mg nebulised)

Hydrocortisone 100mg IV (or prednisolone 40 - 50mg PO) 

Ipratropium Bromide (500 micrograms)

Theophylline (aminophylline infusion 1g in 1L saline 0.5ml/kg/hr)

Magnesium Sulphate (2g IV in NaCl 0.9% over 20 minutes)

Escalate care (intubation and ventilation)

Well's Criteria for probability of Pulmonary Embolism

Don't Die Tell The-Team To Calculate Criteria

Don't = DVT symptoms (3 points)

Die = Diagnosis most likely PE (3 points) 

Tell = Tachycardia (1.5 points) 

The Team = Three days of immobilisation or surgery in the past Thirty days (1.5 points) 

To = Thromboembolism in the past DVT/PE (1.5 points) 

Calculate = Coughing up blood (haemoptysis) (1 point)

Criteria = Cancer (1 point)

Causes of Type 1 Respiratory Failure

Asthma

Pneumonia

Net like lung (Fibrosis)

Oedema - Pulmonary

Embolism / Emphysema

ARDS

Shunt

Small Cell Lung Cancer - Paraneoplastic

Small-LAA (Pronounced 'Smaller')

 

Lambert - Eaton Syndrome

ADH production (ectopic)

ACTH secretion
